I authorize Washington Poison Center, along with its employees, contractors, and representatives, to use The Testimonial in these efforts, including but not limited to websites, emails, letters, social media, brochures, and event materials.

I waive any right to inspect, edit, or approve materials where The Testimonial is used. I also waive any claim to copyright ownership of The Testimonial or related materials, and any claim to financial compensation or gain from its use.

Washington Poison Center may make reasonable edits to The Testimonial for clarity, length, or formatting purposes, while maintaining the original meaning. I agree that my comments may be published as written.
