Please complete the following form for our records. Note that all values are determined by you (the donor). Washington Poison Center is a 501(c)(3) nonprofit organization, and all contributions are tax-deductible to the extent of the law. As the donor, you are responsible for determining the deductibility of the goods or services for your tax purposes.

• For donated goods, provide a description and number of items and the total fair market value of each item (if you have more than four items, please contact us)
• For professional or consulting services, describe the type of service, list the number of hours worked, date(s) of service, and rate of pay
• For facilities, list hours or dates the facility was used, the number of hours, and the published rental rate.
